These medical insurance coverages provide access to a spectrum of health services, from preventive care and prescription drugs to hospitalization and surgery. Regardless of which coverages you choose, you also can purchase coverage for your eligible dependents.
PPO
The Participating Provider Option plan gives you freedom of choice, flexibility, and a broad range of benefit options. With the PPO product, in many cases there’s no requirement to have your care coordinated through a primary care physician. This means seeking a referral to see a specialist or to get a second opinion could become a thing of the past. You may have the ability to choose doctors and hospitals nationwide.
HMO
The Health Maintenance Organization plan gives members access to a broad range of benefits when care is provided by or coordinated through a primary care physician. Female members can also choose a Woman's Principal Health Care Provider.
Individual Medical Insurance
You can apply for an individual medical insurance policy at any time. You (and any eligible dependents you want added to your policy) must meet medical underwriting guidelines, and will have to complete a medical application that may include medical questions. In addition, you may be required to undergo a physical examination to be considered for coverage.
Coverage is available through national and local carriers, including Blue Cross Blue Shield, Aetna, GHI, HIP, Golden Rule, John Alden, Humana, Pacificare, Unicare, Harvard Pilgrim, Celtic, and Tufts HealthPlan. Policy options, premiums, and benefits vary by carrier and location.
Group Medical Insurance
Group medical insurance is available in most locations. The cost of individual and family coverage is based on a group rate by age class. All participants in an age class pay the same amount, regardless of where they live or their medical history. You do not have to submit medical evidence of insurability; however, you must enroll within the enrollment period. Note: This group medical insurance is not available in Wisconsin and Hawaii.
